Our Early Interventionist work with infants and children between the ages of birth to three years of age who are eligible through BabyNet. Early Intervention is designed to improve outcomes for children with developmental delays and disabilities. By providing early, appropriate, and intensive interventions, children and families learn together. Early Intervention is a range of services designed to support a family at the early stages of an infant or toddler’s developmental delay or disability. All of our services through Babynet are FREE!
Our Case Managers work with clients who are eligible through Home and Community -Based Services (HCBS). Case Management is intended to be a collaborative process involving assessment, planning, and advocacy for options and services that address the comprehensive needs of individuals and families through community resources. This approach aims to promote quality and cost-effective outcomes. Eligibility is determined based on Medicaid coverage and individual need.

Case Manager (CM)
CMs work with individuals in the comfort of their homes while receiving the needed support and services they desire. Case managers follow core principles that focus on the individualized care.
