Steps of the Referral Process for Home and Community -Based Services (HCBS)
PRE-ADMISSION SCREENING
In addition to the home care programs, HCBS is also responsible for conducting pre-admission screening for applicants who are accessing Medicaid-sponsored nursing home care. This screening process assures applicants meet the required level of care—skilled or intermediate—for nursing home placement.
HOW TO GET SERVICES
An individual or their family member may make a referral for CLTC services by calling the toll-free HCBS referral number, (888) 971-1637 or by using the HCBS website, https://phoenix.scdhhs.gov/eref/.
